Step-1 (Online Registration)

The electronic version of our registration process is now ready. Please go to the registration site, complete the form, print it, complete the payment information, sign the form and fax or mail it to Diane Gaylord, Conference Secretary. Don't forget to sign and send the form or your registration will not be complete.

What is covered in the Registration Fee

The participant fee includes admission to all scientific sessions and programs, a copy of the Abstract book and Conference proceedings, the Welcome Reception, all breakfasts, breaks, and the Conference banquet. The student registration includes admission to all scientific sessions and programs, the Welcome Reception, all breakfasts, breaks, and the Conference banquet. The Accompanying Persons Program includes the Welcome Reception, a briefing on what to see and do in Albuquerque, and the Conference banquet.

Registration Fees

Please Note: The registration fee is determined by the date the payment is received not the date you submitted your abstract.

* Verification of current student status must be mailed or faxed along with the registration form. This can be a clean copy of your student ID card or letter from current advisor.

Method of Payment

Registration fees can be paid by VISA, MasterCard or American Express credit cards, U.S. Checks from your organization or cash. (Diner's Club and Discover credit cards, personal checks, and wire transfers are not accepted.) A printed receipt will be included in your registration packet at the Conference.

Credit Cards
If paying by credit card, we accept only VISA, MasterCard and American Express. Other credit cards are not accepted. You will receive a confirmation email following the successful submission of your registration.

Cash
Cash will be accepted only at onsite registration at the Conference.

Mailing or Faxing the Registration Form

Be sure the credit card number, expiration date, security code and signature are filled out before mailing or faxing the form. Your registration could be delayed if the form is not completed correctly. Please type or print the information so it is easily read.

On-site Registration

On-site registration will be available with printed registration forms. Early registration fees will not be available at on-site registration.

Cancellation Policy

Cancellation of registration with refund of fee, less a $50 service charge, will be accepted if received in writing by the Conference Secretary no later than June 15, 2005. No refunds will be granted after that date. Refunds will be processed following the conference.
